Documentation you may need to complete your student file and use your benefits at John A. Logan College are listed below.

National Guard/Reservists/Active Duty:

DD214: All service members are given HTH 110 (2) and PED 1000 (2) credits towards their degree once your DD214 is turned in.
Banking Information: If you will be using a military education benefit that pays you a monthly stipend, you will need your banking information for direct deposit. You may also select to receive payments by mail but this is not recommended.
Transcripts: If you have attended a different college, please have your transcripts sent to John A. Logan College Admissions Office. Please let the Veterans Resource Office know that you have transcripts you would like reviewed.
All Eligibility Letters: If you have applied for benefits and have received your eligibility letter, a copy is required for your student file at John A. Logan College.

Spouse of Veteran:

Veteran’s DD214: This is needed to verify the veteran’s SSN for certification purposes. This is also needed for applications that require this documentation.
Veteran’s Disability Letter or Service Connected Death Certificate: This is needed for applications that require this documentation.
Marriage Certificate: This is needed for applications that require this documentation.
